• Go to washburn2.atsusers.com
• Login with user ID and password you created
• Click Athlete Information
• At top of page click the Forms tab
• Click the box in middle of the page
Select a form: 50 State PPE History form, Insurance questionnaire, Medical History Questionnaire, Sickle Cell Policy, Washburn University HIPPA Release Form, Washburn University Insurance Billing, Injury Claims Policies and Procedures and Student annual income and Treatment authorization forms.
Click New
• The forms will load
• All forms must be completed and electronically signed
